Gallstone disease (GSD) is a disease in which gallstones form in the gallbladder and bile ducts. Gallstones can be cholesterol or bilirubin in composition.

Causes affecting stone formation:

  • Age: is a significant risk factor for GSD, with the maximum incidence of clinical manifestations occurring between the ages of 40-69 years.
  • Female gender: Women have a higher risk of developing GSD due to the effect of oestrogens on lithogenic potential. This risk is about 2-3 times higher in women, although the differences flatten with age.
  • Pregnancy: Pregnancy increases the risk of GSD, especially in multiple pregnancies. This may be due to the formation of biliary sludge and stones.
  • Hormone replacement therapy: Postmenopausal hormone replacement therapy increases the risk of GSD problems.
  • Estrogen use: Estrogens may increase the risk of GSD in both sexes.
  • Heredity: An aggravated history of GSD increases the risk of developing the disease.
  • Obesity and hypertriglyceridaemia: Metabolic syndrome including obesity and hypertriglyceridaemia is associated with an increased risk of GSD.
  • Insulin resistance and diabetes mellitus type II: increases the risk of developing GSD.
  • Various liver diseases
  • Use of medications: Some medicines may increase the risk of developing GSD by affecting cholesterol in the bile or bile duct motility.
  • Rapid weight loss and bariatric interventions: These procedures may increase the risk of GSD problems.
  • Total parenteral nutrition: Prolonged total parenteral nutrition may increase the risk of GSD.
  • The epidemic of overweight in children and adolescents: Recent decades show an increasing incidence of GSD in children and adolescents, which is associated with the epidemic of overweight.

The study of the chemical composition of gallstones (lithogenesis) plays a key role in the study of the causes of the development of GSD. It helps to choose preventive measures to prevent recurrence of stone formation and to determine the additional scope of necessary investigations for patients.
