Gilbert's syndrome is a congenital disorder of bilirubin metabolism, usually asymptomatic, manifested by moderate unconjugated hyperbilirubinemia. It can be mistaken for chronic hepatitis or other liver disease. Gilbert's syndrome can affect up to 10% of people.

The pathogenesis may involve complex defects in bilirubin processing in the liver. Glucuronyl transferase activity is reduced, although not as severely as in type II Crigler-Najjar syndrome.

Many patients also have slightly increased red blood cell destruction, but

this does not cause anaemia or hyperbilirubinaemia. Liver histology is mostly normal.

Gilbert's syndrome is most often detected incidentally in young people when elevated bilirubin levels are found, which usually range between 34 and 86 μmol/L and tend to rise during fasting and various stresses.

Gilbert's syndrome is differentiated from hepatitis by bilirubin fractions, which show a predominance of the unconjugated form, as well as normal liver tests and the absence of bilirubin in the urine. The difference from haemolysis is the absence of anaemia and reticulocytosis.

Clinical indications:

Periodic jaundice of the skin and sclera, occurring outside of acute illness and often passing spontaneously. Provoked by fasting or diet, infections (viral, bacterial), stress, menstruation, physical exertion, alcohol consumption.
Asthenic-vegetative complaints without organic lesions: fatigue, sleep disturbances, headaches, irritability (with no objective data indicating liver pathology).
Chronically elevated bilirubin in tests: total bilirubin elevated to 20-60 μmol/l, sometimes higher, direct bilirubin normal or minimally elevated, ALT, AST, GGT, ALP normal.

Anamnestic indications:

Confirmed Gilbert's syndrome in relatives
Episodes of jaundice in the family without hepatitis or haemolytic anaemia
