Penicillin V (phenoxymethylpenicillin) is a natural antibiotic produced by industrial fermentation of Penicillium chrysogenum. Drug therapy through oral administration is the main route of exposure to penicillin V. Secondary routes of exposure are eating animals treated with penicillin and inhaling penicillin dust.

From a clinical point of view, allergic reactions to penicillin can be classified as immediate reactions occurring within one hour after the last injection of penicillin, or delayed reactions occurring at any time from 1 to 48 hours. Immediate reactions are usually mediated by IgE, while delayed reactions are often induced by sensitized T cells. The longer the time before symptoms appear, the more likely it is that a mechanism that is not mediated by IgE is involved.

Immediate reactions are clinically manifested by hives, angioedema, rhinitis, bronchospasm, and anaphylaxis. The main delayed reactions are spotty papular exanthema and delayed hives/angioedema.
